Article summary

When the HAZOP team identified the feasible Causes and likely Consequences it is time to add the Existing Safeguards. The team then needs to decide (with confirmation by subsequent Risk Analyses) whether the Existing safeguards are sufficient, or whether an Action to install an additional safeguard is necessary to reduce the risks to an acceptable level. In this case a new Layer of Protection Analysis (LOPA) can be initiated.

Defining Existing safeguards

For each Consequence (and for each Hazard), define its Existing safeguard(s):

  • Click on the Existing safeguard card next to the Consequence (and Severity).
  • Enter a description for the Existing safeguard. Take note that this is a required field.
  • You can use “Intelligent Text Recognition” to add Equipment to the card. Just start typing the Equipment name in the description field and click <Ctrl> + <Space>. The selected Equipment will be added to the card, but it will also be added as a link in the description field.
  • Click the fold-out bar.
  • Select the Safeguard Type. Note: The Type will be displayed on the card: CB (Control Barrier), or RM (Recovery Measure), or CM (Conditional Modifier).
  • Select the Category, e.g., “SIF Analysis”.
  • Select the applicable Classes: P (People) and/or (Asset) and/or C (Community) and/or E (Environment).
  • Select the Safety Critical Elements/Equipment as applicable. Or click to create a new Equipment. Note: The SCEs are summarized on the Safety Critical Elements Tab .

To create a new Equipment click +.

  • If Not Valid, or Partially Valid, check the box and write a Justification. This can also be done when Consequences are linked to indicate that an Existing safeguard does not apply to both Cause-Consequence combinations.
  • If a Safeguard Type: "Control Barrier" of Category: "SIF Analysis" has been selected, you can select or create a new SIF Analysis. Click SIF Analysis to do so.

    Click P&ID/PEFS to add drawings.
  • Close the details window.

Adding / Copying / Pasting / Deleting Existing safeguards

You can add an additional Existing safeguard for the Consequence, but only when the first Existing safeguards have been defined.

Use the buttons in the Toolbar or the keyboard shortcuts to Add / Copy / Paste / Delete Existing safeguards.
