Witnessing Process for ECHs
The Witnessing process for Schedules also applies for the Condition History records (ECHs) (see the flow diagram above). Note: It is not necessary to mark for both Supervision and Authority. The one can be selected without the other. In this case, the steps that are not applicable (Supervision or Authority) can just be skipped.
Mark Equipment Condition History (ECH) Records for Witnessing
It is advised that an ECH, which has Schedules marked for Witnessing, should be marked for Witnessing as well. However, Witnessing is not automatically applied to an ECH that is credited with a ‘Witness Schedule’.
Each user that have privileges to edit an ECH can mark the ECH for Witnessing. The steps are the same as for Schedules.
To set Witnessing Supervision and/or Authority for an ECH:
- Create a new ECH or Edit an existing ECH.
- Check the boxes for Supervision and/or Authority. Note: On already final Approved ECHs, the checkboxes cannot be checked. Supervision and/or Authority can only be checked when there is a pending event.
- Fill in the rest and click Save.
Final Approve the ECH
Before the Witnessing process can commence, the ECH must be Final Approved.
Supervision - Review the ECH
The Supervision review process for ECHs is similar to the Supervision review of Schedules.
Do Authorized ECH Witnessing
Once the ECH is reviewed successfully (Supervised), the next step is for the Authority to Approve the ECH. This is similar to Schedule Witnessing. Additionally there is a field for the Legal/local Inspection date (see the Table below).
Authority record options for ECHs:
Field | Options |
---|---|
Approval (+ comment) | Approve Disapprove – Always add a Comment. (Note: An additional Approval cycle is needed.) More information required - Always add a Comment. (Note: An additional Approval cycle is needed.) |
NCR | Non-Conformance Report. Once checked, additional NCR fields appear. In Shell this must conform to BBS 742.05.2755. A comment with additional information on the NCR is important. |
NRC Impact | Minor - No risk to the integrity of pressure Equipment or the coherence of the management system. Major - Risk for the integrity of pressure Equipment and/or the coherence of the management system. Attention – Not used everywhere |
NCR Closed | This should be checked when the Non-Conformance Report is closed. The Authority must use the comment to specify the requirements and evidence from the NCR, to support the closing of the NCR. |
Legal/local Inspection date | This date can either be part of the ECH Authority Form or the Supervision Form. This can be set with an application setting. |
For Shell this means:
- Include RIG Pdf in ECH.
- Submit RIG Pdf to DocData for archiving in AIM.
- Store a hardcopy of the report in STW.kmr archive.
