The IMS Dynamic Form is a configurable data collection tool. In terms of Checklists, Dynamic Forms provide you with a more flexible approach for collecting data in Equipment Condition Histories.

With Dynamic Forms it is easy to configure your own Forms, e.g., an Inspection Checklists to be used within the ECH. This way you can create the perfect inspection workflow. You can either create a new Dynamic Form or edit an existing Dynamic Form by creating a copy or new iteration.

When a Dynamic Form has been created and approved, it is ready and available to be used. For instance, all created Checklists will be available in the ECHs (depending on the Form's configured conditions) for data collection. You can also review collected data in multiple ways.

Level

Description

Form

The Form is the highest level. On this level the Form Type, e.g., "Checklist", or "Schedule Scope, i.e. Plan Codes", is specified. The Lookup and Lookup Lists available to the Data Templates are also defined on this level. 

Stage

A Form can have multiple Stages. Each Stage has a Sequence number that determines the order in which they are shown in the Form.

Section

A Stage can have multiple Sections . Each Section has a Sequence number that determines the order in which they are shown in the Stage. The Section's Style determines how the Section's Data Fields (Question and Answers) will be displayed, e.g., "Radio Buttons", "Long Answers", etc.

Field

The Field is typically used to display and collect data, e.g., it can display a question and collect its answer, or display an image, etc. It is the most important layer of the Form.


Each Section can have multiple Fields. Each Field has a Sequence number that determines the order in which they are shown in the Section. Fields can also be grouped together via Categories.
All the Field's properties like Help Text, Description, Observation, Control Type, etc. is defined here. The Control Type determines the type of data to display/collect: e.g., "Text", "Numeric", "Signature", "Image", "Option List", etc. A Template with predefined properties can also be used to help define the Field.

When a Form is used to collect data, IMS will keep track per Stage of the number of Sections completed, as well as the Issues recorded.

Let's start by configuring a new Dynamic Form.
