Creating and Linking Anomalies

This section of the Handbook explains how to Create and Link Anomalies in IMS.

Anomalies can be created or linked from several different locations throughout IMS. Regardless of of the locations where Anomalies are created, the underlying creation flow remains constant throughout IMS.

Shared Anomaly Creation Workflow

Regardless of the starting point, the process of creating a new Anomaly follows the same general steps:

  1. From the source screen, use the Anomaly action (button or toolbar action).

  2. A dialog opens, listing Anomalies already linked to the same Equipment.

  3. Anomalies already linked are pre-checked.

  4. Select an existing Anomaly to link it, or click Create new anomaly to open the creation form with the Equipment pre-filled and locked.

  5. Fill in the required fields and save.

The required fields on the Anomaly creation forms include: Short Description, Anomaly Type, Anomaly Status (defaults to Open), Identified Date (defaults to today). Equipment is always pre-filled and cannot be changed.

When a new Anomaly is created, IMS automatically generates its first Assessment (revision 0), pre-set to the Monitor response with the description “Initial assessment”, and dated to the creation date.

Context-specific Anomaly creation workflows are described below.

Create an Anomaly from the Equipment Details Page

On the Equipment Details page, the Anomalies tab lists every Anomaly linked to the Equipment with a count badge.

To create an Anomaly:

  1. Select the Anomaly Tab.

  2. Click + to open the Anomaly creation dialog.

  1. Fill in the details and click Save.

The Details Page for the newly-created Anomaly will open in your browser.

Create an Anomaly from an ECH

On an ECH, Anomalies can be created from one of two locations:

  1. From a Dynamic Forms Checklist (PEI, CIvil, PLSS) or Inspection Codes (PLSS).

  2. From the Created Schedules Section of the ECH Details Page.

Create an Anomaly from a Checklist/Inspection Code

On a Checklist Observation flagged as an issue, a Track as anomaly option links or creates an Anomaly.  

The finding text is carried onto the Anomaly and the source observation remains visible on the Findings tab of the Anomaly Details Page. To create the new Anomaly:

  1. Click Create Anomaly.

  1. Fill in the Anomaly details in the Anomaly dialog.

  2. Click Save.

The newly-created Anomaly’s Details Page will open in your browser.

Notes

The option to create an Anomaly from a Dynamic Form appears only on those Checklists configured for Anomalies.

Create an Anomaly from the Created Schedules Section

For PEI, Civil, and PLSS, a Create Anomaly toolbar button is now also available directly in the ECH's Schedules Created section header (shown only to users with Anomaly creation rights), as a faster path than going through a Conditions Found observation first.

To create an Anomaly from the Schedules Created section:

  1. On the ECH Details Page, go to the Schedules Created section.

  2. Click the Create Anomaly button in the section header.

  1. The usual Anomaly creation dialog opens, pre-filled with the ECH's Condition Date as the default Identified Date.

  2. Fill in the remaining details and click Save.

Note

This method of creating an Anomaly is a shortcut to the same underlying creation flow described above, not a separate mechanism.  It can be useful when a new Anomaly needs to be created against an ECH without filling in the complete details of a Checklist.

Adding a Subsequent Finding to an Existing Anomaly

From the ECH Conditions Found section, select the existing Anomaly from the list instead of creating a new one, so the new Finding is added to the existing record rather than creating a duplicate.

Anomalies can be linked directly to one or more Circuits or Measurement Sets, from those entities' own Details Pages, without going through an ECH observation first. This is useful when a condition is identified during data review or trending rather than during a fresh inspection.

To Create and Link a new Anomaly to a CML directly from a Circuit:

  1. Navigate to the CML Measurement History tab on the Circuit’s Details Page.

  2. Select one or more CML rows and use the Create Anomaly button to open the Anomaly Selection Dialog.

  1. Click + open the Anomaly creation dialog, or select one or more Anomalies to link the CML Measurement History to.

  1. Fill in the required fields for a new Anomaly and click Save.

The Anomaly grid will show the previously selected CMLs as linked to the newly-created Anomaly.

Note

For Anomaly creation from the CML Measurement History tab to work, all of the selected CMLs must belong to the same piece of Equipment.

Linking CMLs to Existing Anomalies

To link an existing Anomaly to CMLs directly from a Circuit:

  1. Navigate to the CML Measurement History tab on the Circuit’s Details Page.

  2. Select one or more CML rows and use the Create Anomaly button to open the Anomaly Selection Dialog.

  1. Select an existing Anomaly from the Anomaly grid.

  2. Click Save.

The selected CMLs will be linked to the selected Anomaly.

An Anomaly can be created or linked to from the Measurement Set details screen. To open the Anomaly dialog:

  1. Select one or more entries from the Measurement Set grid.

  2. Click the Apply Anomaly to Measurement Set button.

  1. Select an Anomaly to link to from the Anomaly grid.

  2. Click Save to apply the changes.

If you need to create a new Anomaly, follow the instructions described in the previous section.

An Anomaly can be created from the S-IDAP Screen as well. To create an Anomaly from the S-IDAP screen:

  1. Open the Measurements Tab of the Trending & Corrosion Analysis tab in S-IDAP.

  2. Select the desired CML.

  3. Click the Apply Anomaly to CML(s) button.

Follow the instructions from the previous section to Apply an existing or new Anomaly to the CML.

Create Anomalies via Import

Anomalies and Assessments can be created and updated through the standard IMS bulk import mechanism, including their risk matrix selections.

When a Dynamic Form (D-Form) import creates or updates a PEI observation that is flagged as trackable, IMS automatically links or creates the associated Anomaly and reports how many were affected in the import summary.
