You can create a new LOPA directly from the LOPA Main Screen. or from the HAZOP itself. In both cases you can link it to a HAZOP Consequence.
Creating LOPAs from the LOPA Main Screen
To create a LOPA:
Go to the IMS SIS Main Menu.
Click LOPA to go to the LOPA list (main screen).
Click + to create a new LOPA.

Go to the LOPA main menu and click +.
Enter the required data in the popup window.
Click Save when done.

Description | Enter a description for the LOPA. |
HAZOP / Node | If a HAZOP Consequence has been selected, then the associated HAZOP/Node will be filled in here automatically. |
Hazard | If a Consequence has been selected, the associated Hazard (and its Severity) will be filled in here automatically, if applicable. Else, you can enter it here. |
The new LOPA will be created, and you will be redirected to the detail page of the LOPA that was just created.
If an existing Consequence from a HAZOP was selected: The Consequence and its Severity will be copied, the Cause(s) (and Frequencies) will be copied as the Initiating Event(s), the Safeguards will be copied as Barriers, and the ALARP notes will be copied. These objects in the HAZOP and LOPA will remain synced to each other. See Syncing HAZOPs with connected LOPAs.
Creating LOPAs from the LOPA HAZOP Details Page
A LOPA can also be created directly from any Consequence card on the HAZOP details page.
To create a LOPA:
Select the desired Consequence.
If it has already been indicated that a LOPA is required, click Create LOPA on the associated Severity card.
If you first need to specify the LOPA requirement click the dropdown to learn how to do this:
Click on the Consequence card fold-out.

If the Severity of the Consequence has been estimated (i.e., a Risk Assessment has already been done), you will see a LOPA checkbox.
Check the box.

You should now see the Create LOPA button. It will show on the Consequence fold-out as well as in the Severity column.

Click the button.
First you can decide if you want to link the Safeguard and new LOPA’s Barrier to an Existing Barrier that is already being used in another LOPA. If so, click Link Barrier. Note: This link can also be established before the LOPA is created via the Safeguard card or it can be done at a later stage when you further define the LOPA Barriers during the LOPA session.

After the Barrier is linked (or if not desired) click Next. Note: When you click next Safeguards that are not linked to an existing Barrier will automatically be linked to newly created Barriers.
Enter the LOPA Name.
Enter the LOPA Description (If not yet done).
Click Save when done.
The new LOPA will be created, and you will be redirected to the detail page of the LOPA that was just created.
The Consequence and its Severity will be copied, the Cause(s) (and Frequencies) will be copied as the Initiating Event(s), the Safeguards will be copied as Barriers, and the ALARP notes will be copied.
